Job Description

We are seeking a dedicated and compassionate School-Based Licensed Practical Nurse (LPN) to provide one-on-one medical and educational support to a single student. In this rewarding role, you will accompany the student throughout their school day. Including on the bus, in class, and during school activities, to ensure their medical needs are met. Beyond clinical nursing, you will serve as a vital bridge, collaborating with teachers and therapists to help the student safely and fully participate in their educational environment.

Schedule: 6:45am-1:45pm / Frontier High School

Responsibilities

• Monitor the student's health status closely throughout the entire school day, providing immediate interventions according to their Individualized Healthcare Plan (IHP).
• Assist the student in navigating the school environment, modifying classroom materials as directed by the teacher, and helping them stay engaged in learning activities while safely managing their health.
• Safely administer scheduled and/or emergency medications as needed per their healthcare plan under the direction of an RN/Medical Director and document care when necessary.
• Accompany the student on any school-sponsored field trips/programs to ensure continuous safety.
• Coordinate seamlessly with the School Nurse (RN) and/or Medical Director, classroom teachers, special education staff, and the student's parents to provide updates on health and classroom progress.

Job Qualifications

 

Qualifications & Requirements

• Active, unencumbered Licensed Practical Nurse (LPN) license in the state of NY.
• Current Basic Life Support (BLS) / CPR certification.
• Previous pediatric, home health, or school nursing experience is highly preferred.
• Strong clinical judgment, patience, flexibility, and a passion for working with children with special needs.
• Must pass all state-required school background checks and fingerprinting.
